Requesting Your Health Care Records
You have the right to request access to your health care records from The Space Between Therapy.
To request your records:
- Please submit a written request to: [email protected]
- Include the client’s full name, date of birth, and a brief description of the records you are requesting.
- A signed authorization form may be required before records can be released.
- Requests are processed in accordance with applicable state and federal laws and within required timeframes.

Contacting the Texas Behavioral Health Executive Council
The Texas Behavioral Health Executive Council oversees the licensing of behavioral health professionals in Texas.

Filing a Consumer Complaint
Clients may file a complaint with the Texas Office of the Attorney General’s Consumer Protection Division if they have concerns related to services provided.
